Heart de Jesus is a young girl who grew up not knowing he biological parents, though not wanting in love under the care of Jude (Zanjoe Marudo) and Clara de Jesus (Bella Padilla). Despite being poor and having a rare heart condition, Heart lives happily and contentedly with her family. But later on, she becomes comatose and by some miracle, her soul becomes visible, only to her doctr, Margaret Divinagracia (Coney Reyes). They become close over time, with Dra. Margaret eventually finding out that Heart is her real granddaughter.
Towards the end, both Dra. Margaret and Heart were seen battling for their lives. When heaven came calling, Dra. Margaret was given a choice to either save herself from danger or make the ultimate sacrifice so her grand daighter can live again.
